As members of a High Reliability Organization (HRO), every employee should practice CHAMP behavior and strive to make patient safety a top priority. Manages and leads programs, activities and personnel related to Clinical Pharmacy Services, Medication Safety and Drug Expense Oversight for a 5-hospital health system. Interfaces with Nursing and Medical Leadership on issues of Medication Safety, Medication-related regulatory compliance and drug utilization. Coordinate all clinical pharmacy services and programs ensuring that the these services promote desirable patient outcomes through the use of safe, appropriate, and cost-effective medication therapies. Oversees clinical managers across the system for both Ambulatory and Acute Care Clinical Pharmacy Services promoting standardization and advancing clinical pharmacy practices. Responsible for advancing clinical pharmacy services in relation to system pharmacy performance metrics including Patient Experience and Clinical Processes of Care and Outcomes.
- 1. Assists the Director of Pharmacy in setting priorities for and tracking progress on quality, safety, and regulatory compliance. Maintains expert working knowledge of The Joint Commission on Accreditation of Healthcare Organizations (JCAHO), National Patient Safety Goals (NPSG), Department of Public Health (DPH), American Society of Health system Pharmacists (ASHP), Institute for Safe Medication Practices (ISMP) and Centers for Medicare/Medicaid Services (CMS) medication-related and pharmacy practice standards and metrics and relates them to current departmental and organizational policies and procedures.
- 2. Manages the clinical services and all coordinator /specialist staff to ensure effective and cost -efficient formulary management. Involved with planning and execution of staff development programs, in-service education, and competency assessment of staff on performance, behavioral, drug utilization, safety and regulatory issues. Improve patient outcomes by identifying opportunities to improve clinical quality of medication use through appropriate prescribing and monitoring that minimizes /mitigates patient risk and/or enhances ability to meet quality outcomes/core measures.
- 3. Directs activities of the Drug Information Service including qualitative analysis of calls, adverse drug reactions reports, and assistance on medication variances, ensuring data are collected, analyzed and distributed in accordance with departmental and hospital policies and goals. Oversight to the drug information service and personnel. Provides direct oversight and management of drug shortages and recalls, ensuring appropriate action plans (e.g. substitutions, drug use guideline changes, communication) are executed.
- 4. Participates in the department's educational affiliations with external academic institutions. Identifies targets for and participates in in-service education for medical, nursing, pharmacy and support staff aimed at correcting problems in drug use at Yale-New Haven Hospital.
- 5. Fosters open inter- and intradepartmental communication between staff and managers. Participates in departmental, organizational and health system committees related to safety and compliance performance as directed by the Director, Pharmacy Services.
- 6. Contributes to the effective operation of the department by enforcing and complying with institutional and departmental policies and procedures.
- 6.1 Effectively emulates the principles and conduct consistent with a High Reliability Organization.
- 7. Administers human resources policies and procedures in accordance with hospital and departmental policy.
- 8. Communicates the status of projects, operations and staff concerns, and problems to the Director on a weekly basis or more frequently as needed.
- 9. Complies with all aspects of the hospital's patient/customer relations program. Demonstrates professionalism in all activities, as measure by a concern for patient care, the ability to listen /cooperate, showing flexibility under stress, the ability to help others, and the ability to set and achieve goals.
- 10. Manage drug expense to budget by monitoring drug utilization, identifying opportunities for improvement , and initiating cost avoidance programs to achieve planned savings and /or cost avoidance. Strategically plan future budgets through drug forecast, volume, and inflationary projections.
- 11. Establish a succession process/plan for development of leadership positions and increased staff engagement that improves communications, builds teamwork, and optimizes job functions to maximize efficiency.
- 12. In the absence of the Director of Pharmacy, performs all duties of the Director of Pharmacy to ensure safe medication management processes overall.
Graduate of an accredited College of Pharmacy. Advanced degree (M.S. or Pharm.D.) and completion of an ASHP Accredited Pharmacy Practice Residency Program is required, plus a minimum of 4 years of clinical pharmacy managerial/supervisory experience overseeing pharmacists who are involved in providing clinical pharmacy services, making formulary decisions and providing medication management. Experience leading a large clinical pharmacy services department in an academic medical center is preferred.
Minimum of 4 years management experience in a healthcare setting , preferably academic medical center, with strong working knowledge of formulary management, clinical pharmacy program development, pharmacy metrics, compliance and regulatory issues relative to hospital pharmacy practice, physician engagement, and project management. Candidates with experience managing many diverse projects and programs related to clinical pharmacy that have demonstrated positive results on budget mitigation and patient outcomes are required. Clear demonstration of professional personnel performance management is also required.
CT Pharmacist Licensure eligible
Six Sigma or other validated quality improvement certification is preferred. . Project management, problem solving skills are required and must be demonstrated through past experience. Strong interpersonal skill and the ability to coordinate work between diverse groups are essential. Knowledge of automation technologies pertinent to medication use is strongly preferred.
