ADVANCED PRACTICE REGISTERED NURSE OR

PHYSICIAN ASSISTANT

WALK IN & TELEMEDICINE

LOCATION(S): SHELTON & WEST HAVEN CT

SCHEDULE: PER DIEM, AS NEEDED, DAYS, EVENINGS WEEKENDS

On-demand virtual clinic provides a unique, technology enabled experience of personalized healthcare to patients around CT.  Based on exceptional standards of clinical care at YNHH, on-demand virtual clinic is fostered by an environment of continuous learning and teamwork. Candidates must have an appreciation for the patient experience and be able to integrate clinical practices and technology to provide high quality clinical care. On-demand virtual clinic offers the following clinical shifts 7:00 a.m. -7:00 p.m.

An integrated clinical EMR system (Epic) is used for all clinical tasks and documentation. Candidates can expect to play an active role in the development and implementation of telemedicine practice pathways, as well as in clinical optimization initiatives. All providers are covered by YNHH professional liability malpractice insurance.

Responsibilities

ESSENTIAL FUNCTIONS

  • Diagnose and treat appropriate patients via 2 ways video chat
  • Coordinate care for all patients seeking help at on-demand virtual clinic including, but not limited to, referral for higher acuity clinical settings and post visit follow up
  • Provide appropriate treatment and follow up recommendations
  • Clearly explain treatment recommendations and follow-up plans both verbally during encounter and in writing after the visit
  • Focuses on quality of care and the patient experience including timeliness and effective communication.

 

Qualifications

EDUCATION AND REQUIREMENTS

  • Educational preparation at the Master's Level of nursing. Graduation from an accredited Advanced Nurse Practitioner program required. In addition, holds a BSN degree from an accredited institution.  
  • 3 years of APRN or Physician Assistant experience in Internal medicine, Urgent Care, Primary Care, Family medicine clinical experience
  • Current DEA and CT license; will need to pursue licensure in NY, RI, and MA
  • Superior verbal and written communication skills
  • Willingness to engage patients and practice medicine through telemedicine
