The Physician Assistant for the Pulmonary Hypertension Program will serve as an integral member of a larger interdisciplinary team contributing to excellence in patient care, research and teaching. Participates in Hospital and Health System teams aimed at improving clinical care at YNHH and YNHHS. Facilitates engagement of physician and nursing leadership with strategic initiatives. Reporting to the Ambulatory Medicine Director and /or Clinical Program Director working collaboratively with the Pulmonary Hypertension Program Director, physicians, Advanced Practice Providers (APPs), and staff caring for patients at the Winchester Chest Clinic and Yale New Haven Hospital to ensure goals and objectives of the Pulmonary Hypertension program are successfully met. Responsible for collaborating with healthcare team throughout the continuum promoting high quality, comprehensive care to patients on the Pulmonary Hypertension Service. Assists with program development and strategic planning associated with the clinical program. Assists with the oversight of regulatory and clinical activity and ensures compliance with CMS , Joint Commission, state and national regulatory and clinical standards. Provides oversight and coordination of all elements of the Pulmonary Hypertension Association accreditation process.
1. Clinical Responsibilities Supports Ambulatory Medicine Director, Clinical Program Director, and Pulmonary Hypertension Program Director with ongoing management of the Pulmonary Hypertension Program.
2. Professional Development: Assists leadership in the development of program goals and objectives.
3. Quality improvement, regulatory requirements, Pulmonary Hypertension Association Accreditation and initatives associated with the Pulmonary Hypertension Program at YNHH.
4. Leadership Responsibilities: Communicates and works with physicians and clinical staff to assess educational needs of patients , families, community and staff.
Graduate from an accredited Physician Assistant Program
3-5 years Physician Assistant experience in an acute care environment with experience in the management of pulmonary patients and/or critical care. Experience in collaborating with members of the care team to coordinate specialty, complex patient needs is required. Experience in clinical process improvement, outcomes management , case management, and physician relations; experience in procedures preferred. BLS and ACLS required.
SPECIAL SKILLS: Excellent clinical and decision making skills.
Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA) Connecticut State Physician Assistant License.
3-5 years APP experience in an acute care environment with experience in the management of pulmonary patients and/or critical care. Experience in collaborating with members of the care team to coordinate specialty, complex patient needs is required. Experience in clinical process improvement, outcomes management, case management, and physician relations; experience in procedures preferred. BLS and ACLS required.
