Overview

Join our growing Palliative Care service in Southwestern Connecticut. This position will provide social work services in our brand new oncology focused outpatient clinic. You will join a respected Palliative Care service line as we grow to support our patient and family needs in the ambulatory setting. The new outpatient clinic has full interdisciplinary team support, including physician, RN care coordinator, chaplain, and practice supervisor.

Responsibilities

  • This Part Time (0.6 FTE) LCSW role will include:

    • Perform intake and follow up social work assessments for clinic patients and families
    • Individual therapy for select patients and families based on needs identified
    • Assistance with coordination of social services as needed for patients, including applications and referrals for community based resources.
    • Bereavement screening and coordination with inpatient social worker and geriatric LCSW in the bereavement program.
    • Education of patients and families about goals of care and advanced care planning, including assistance with document completion.
    The Section of Palliative Medicine works in close collaboration with a large Section of Geriatric Medicine, including a Geriatric and Palliative Care LCSW (outpatient) and Palliative Care MSW (inpatient). The section provides education to residents, fellows, nursing students, and social work students from many surrounding institutions including Yale. This team was recently recognized for the 2018 Empathy Amplified Award from the Cleveland Clinic for innovative.

Qualifications

EDUCATION

Master's Degree from an accredited school of social work.

 

EXPERIENCE

Qualifying social work experience for licensure consists of a minimum of 3,000 hours of post-masters social work.

 

LICENSURE

Licensed Clinical Social Worker (LCSW) from the State of Connecticut, or those holding licenses from other states, will need an 'endorsement' and meet the State of Connecticut license requirements.,

 

SPECIAL SKILLS

Ability to perform independently and provide clinical practice in a health care setting. Computer experience and knowledge of word processing, spreadsheet, and data entry systems.

 

PHYSICAL DEMAND

MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap and influenza vaccination are strongly encouraged.
