Under the direction of the Manager of the Winchester Chest Clinic working collaboratively with the Pulmonary Hypertension Program Director and the Ambulatory Medicine Leadership assumes clinical and administrative responsibilities for the Pulmonary Hypertension Program at YNHH. The Nurse Coordinator maintains oversight , coordination, and evaluation of the Pulmonary Hypertension Program supporting education, clinical practice, and quality improvement. Serves as a role model for inter-professional communication and exemplifies organizational leadership. The Nurse Coordinator for the Pulmonary Hypertension Program is accountable for strategic program development, implementation, evaluation, and provides oversight and coordination of all elements of the Pulmonary Hypertension Association accreditation process. The Nurse Coordinator maintains communication with patients , families, staff and providers in order to ensure coordination of patient care across the continuum.

Responsibilities

  • 1. Provides advanced clinical leadership and maintains responsibility for all clinical and administrative aspects , initiatives, and processes associated with the Pulmonary Hypertension Program at YNHH.
    • 1.1 Provides vision and leadership in a collaborative environment that offers job satisfaction , recognition, and stimulates innovative thinking to accomplish goals and objectives. Collaborates with leadership in the development of objectives, goals, and strategic plans for the Pulmonary Hypertension Program and develops actions plans to accomplish set goals and plans.
  • 2. Demonstrates exceptional knowledge and skills that reflect expertise in communication, customer service , professionalism, problem solving, resource management, teamwork, and leadership.
    • 2.1 Ensures a professional work environment and maintains responsibility for professional practice and individual professional development.
  • 3. In collaboration with leadership, maintains responsibility for the implementation of the Pulmonary Hypertension quality improvement program.
    • 3.1 Maintains oversight of the coordination and monitoring of pulmonary hypertension processes at YNHH appropriately incorporating identified opportunities into performance improvement initiatives.
  • 4. Provides leadership, expertise and support in planning, promoting, designing, and ensuring delivery of education to nurses, physicians, residents, support staff, patients, families, and community based on best practice and YNHH's strategic goals and initiatives.
    • 4.1 Communicates and works with physicians and clinical staff to assess educational needs of patients , families and community.
  • 5. Provides oversight and coordination of all elements of the Pulmonary Hypertension Association accreditation process.
    • 5.1 Responsible for collecting and analyzing data related to pulmonary hypertension measures and metrics.

Qualifications

EDUCATION

 

Bachelor's Degree in Nursing is required. Masters Degree is preferred. Current in BLS and ACLS.

 

EXPERIENCE

 

Bachelor's Degree in Nursing is required. Masters Degree is preferred. Current in BLS and ACLS.

 

LICENSURE

 

Current CT State Nursing license required.

 

SPECIAL SKILLS

 

Knowledge of pulmonary physiology and pulmonary hypertension disease process. Must have excellent clinical and decision making skills and understanding of organizational processes and dynamics. Must have high level of personnel management, ability to make independent judgments and implement required changes. Must be a change agent and possess excellent interpersonal, communication, organizational, and leadership skills. Must be self motivated, goal oriented, and able to work independently. Must have the ability to work cooperatively and effectively with physicians, clinical staff, hospital departments, and outside organizations utilizing effective verbal and written skills. Must be proficient with computer based applications. Ability to manage multiple priorities and meet specific deadlines.

 

Additional Information

Bachelor's Degree in Nursing is required.  Master's Degree is preferred. Current in BLS and ACLS. Knowledge of pulmonary physiology and pulmonary hypertension disease process.
