Overview
Responsibilities
1. Provides clinical social work services which may include crisis intervention, advocacy, bereavement counseling, information and referral, behavioral intervention to patients/families/significant others.
1 1 Interviews patients, families, or significant others upon referral by Care Coordinator, or hospital staff.
1 2. Referrals include, but are not limited to the following issues:
1 4. Develops a psycho-social assessment of the impact of the identified problems in the patient.
1 5. Documents their initial assessment in the electronic medical record in accordance with accepted professional and hospital standards as measured by supervisory observation.
1 6. Implements plan in accordance with accepted professional and hospital standards as measured by supervisory observation and feedback.
1 7. Demonstrates and maintains current knowledge to address cultural issues as observed by supervisor and as indicated by feedback.
1 8. Demonstrates and maintains current knowledge and skill in providing appropriate care for all patient populations as observed by supervisor and as incicated by feedback from staff.
1 9. Develops plan for social work intervention in response to assessed needs and in cooperation with patient/family/significant others in accordance with accepted professional and hospital standards as measured by supervisory observation and feedback.
2 0 Gathers information regarding patient's problems which impact on patient's condition with accuracy as measured by supervisory observation.
Documents reassessments in the electronic medical record for ongoing cases as indicated ut no less than weekly. |
Maintains effective, open communication with all members of the health care team and other hospital staff.
2 1 Functions as an active, contributing member of an inter-disciplinary team as measured by feedback from health care team.
2 2. Interacts with co-workers, physicians, and other team members in a positive, cooperative manner as measured by supervisory observation and feedback.
2 3. Demonstrates collaboration with other disciplines in service provision as measured by supervisory observation and feedback.
Maintains appropriate patient records as mandated by department policies.
3. Completes written report in EMR of evaluation and plan for patients' chart on the day of initial consultation as indicated by review of charts and supervisory observation.
3 1. Performs activities in compliance with JCAHO and departmental standards as indicated by monthly random review of medical records and social work materials.
3 2. Documents activities and treatment planning in compliance with departmental and J.C.A.H.O. standards.
Develops and maintains positive working relationships with community agencies/facilities and service providers to enhance the availability of resources and to facilitate patient access.
4. Demonstrates knowledge of community resources and utilizes them effectively as observed by supervisor.
Maintains necessary written and verbal communications with community agencies and facilities as measured by supervisory observation and feedback.

4 1 Demonstrates knowledge of department/hospital programs policies and procedures.
4 2. Identifies problem areas or gaps in services in hospital and department in accordance with professional standards measured by supervisory
observation.
5 Accountable for following hospital policy and procedures, NASW Code of Ethics, and any other Hospital, government and/or regulatory policies, rules or regulations.
5 1. Makes effective recommendations for developing solutions to identified problems or service gaps as measured by supervisory observation.
Professional development.
1. Attends educational seminars to maintain and meet expectations set forth by Hospital and departmental standards.
2. Demonstrates and maintains current knowledge of and skill in providing appropriate care for patients, as observed by supervisor and as indicated by feedback from staff.
Qualifications
EDUCATION
Master's Degree in Social Work.
EXPERIENCE
Experience in health care setting and crisis intervention skills preferred.
