Job Description

Overview

The MFM nurse midwife is an integral part of the women?s health clinical team and provides leadership in health care on behalf of the woman and families in our ambulatory clinic. The CNM evaluates, treats, and manages the care of the high risk pregnancies (which include diabetics and postpartum patients) of the MFM practice. The CNM would serve as a member of the prenatal diabetes program, and perform routine and follow up care. These visits include review of daily glucose values, counseling on diabetic concepts in accordance with the requirements of ADA Recognized Diabetes Program, and review of routine prenatal care topics. The CNM will work collaboratively with our physician staff and our Certified Diabetes Nurse Educator(s). Community activities are required in addition to collaborating with advocacy groups and local agencies.

Responsibilities

  • 1. The CNM provides full scope midwifery care as outlined in the Core Competencies for Basic Midwifery practice developed by the American College of Nurse-Midwives in outpatient settings: A. Independently manages primary health screening and health promotion of women from prenatal to postpartum. 1. Provides DSMT and medication management as needed in consultation with the providers of the MFM practice under the ADA Recognized Diabetes Program. 2. Identifies health care needs and assesses glycemic control of patients during the course of their pregnancy and in the postpartum period. 3. Focuses on continuing clinical care to the postpartum patients. This would include the comprehensive postpartum visit with discussion on contraception, lactation, and health care maintenance. 4. Perform IUD and Nexplanon insertions. 5. Management strategies and therapeutics during the postpartum period for mental health and blood pressure checks. 6. Allow for the expansion of postpartum services to be compliant with ACOG recommendations include a postpartum evaluation within 21 days of delivery and to achieve Medicaid pay for performance milestones. B. Management of Common Health Problems Independently manages infections, self-limited conditions, and mild and/or stable presentations of chronic conditions, utilizing consultation, collaboration, and/or referral to appropriate levels of health care services as indicated. 1. Applies the knowledge of midwifery practice that includes, but is not limited to, the following: a. Identification of deviations in the following areas: Cardiovascular/hematologic Dermatologic Endocrine Eye, ear, nose, and throat Gastrointestinal Mental health Musculoskeletal Neurologic Respiratory Renal b. Management strategies and therapeutics for the treatment of common health problems/deviations of essentially healthy women
  • 2. Care of the Childbearing Woman: Independently manages the care of women during pregnancy, childbirth, and the postpartum period Applies knowledge of midwifery practice in the antepartum period that includes, but is not limited to, the following: 1. Confirmation of pregnancy 2. Genetics, placental physiology, embryology, and fetal development 3. Epidemiology of maternal and perinatal morbidity and mortality 5. Influence of environmental, cultural and occupational factors, health habits, and maternal behaviors on pregnancy outcomes 6. Emotional and psychosexual changes during pregnancy 7. Health risks, including but not limited to, domestic violence, infections, and substance use/abuse 8. Promotion of breastfeeding 9. Indicators of normal pregnancy and deviations from normal 10. Assessment of the progress of pregnancy and fetal well-being 11. Etiology and management of common discomforts of pregnancy 12. Management strategies and therapeutics that facilitate healthy pregnancy 13. Deviations from normal and appropriate interventions including management of complications and emergencies 14. Anticipatory guidance related to birth, breastfeeding, parenthood, and change in the family constellation
  • 3. 1. Care of Mother Applies knowledge of midwifery practice in the postpartum period that includes, but is not limited to, the following: A. Postpartum self-care, newborn care and feeding, contraception, and family relationships C. Management strategies and therapeutics to facilitate a healthy puerperium D. Facilitation of the initiation, establishment, and continuation of lactation E. Deviations from normal and appropriate interventions including management of complications and emergencies F. Management of discomforts of the puerperium 2. Care of Newborn Applies knowledge of midwifery practice to the newborn that includes, but is not limited to, the following: A.Effect of maternal/fetal risk factors on the newborn B. Bonding and attachment theory
  • 4. Medical Record: 1. Prepares appropriate patient records as mandated by JCAHO 2. Prepares medical record which summarizes assessment, diagnosis, and outlines treatment program and disposition as determined by chart review.
  • 5. Medical Education 1. Participates in active continuous medical education A. Acquires at least 50 hours of CME per year 2. Participates in departmental education activities, journal club, and ground rounds 3. Participates in hospital medical education program 4. Effectively teaches by discussion and example medical students, physician assistants, physicians, student midwife students, APRN students assigned to OB/GYN department for educational purposes. A. Presents cases of special teaching value B. Assists in evaluation of trainers as require
  • 6. Participates in marketing strategies to expand visibility within and outside of the hospital community.
  • 7. Research: 1. Participates, collaborates, or initiates research activities within the department of Women?s Services designed to improve the health care of women and infants 2. Applies research-based data to clinical practice in the care of obstetrical and gynecological patients 3. Educates staff in regards to research based initiatives

Qualifications

EDUCATION

 

requires both undergraduate and Nursing master?s degree

 

EXPERIENCE

 

Three (3) to five (5) years midwifery experience in culturally diverse settings; experience with diabetes management preferred.

 

LICENSURE

 

Registered nurse licensure; Licensed Nurse Midwife license, requires both undergraduate and master?s degree

 

SPECIAL SKILLS

 

Knowledge of diabetes in pregnancy and postpartum. Excellent interpersonal and communication skills. Knowledge of research methods and computer skills required.

 

PHYSICAL DEMAND

 

May require travel to other MFM locations within YM/YNHHS
