Job Description

Provide primary nursing care and case management services to pregnant women, postpartal women, infants, and preschool children. Directly responsible to the Supervisor of Clinical Services.



  • 1. Completes health, developmental, and social assessment of every mother and baby admitted to care.
  • 2. Provides health education including healthy lifestyles for women and children, childbirth education, parenting skills, care of infants and children.
  • 3. Provides direct nursing care to mothers and children with medical problems.
  • 4. Provides case management services including referrals and coordination of care.
  • 5. Works with Lawrence and Memorial Obstetric Clinic Nursery and Community Health Clinic to identify target population and coordinate care.
  • 6. Documents services and coordination in clinical record.
  • 7. Supervision of Maternal Child Health Outreach Worker.
  • 8. Assumes responsibilities to participate in at least 12 hours of education per fiscal year.
  • 9. Demonstrates proficiency using, interpreting, and testing blood glucose monitoring equipment.
  • 10. Performs other duties as assigned by Supervisor of Clinical Services.
  • 11. Projects a positive attitude and enthusiasm about agency mission, philosophy, and policies.
  • 12. HIPAA Access Level 4.




B.S.N., preferred, or R.N., current Connecticut RN license and at least 1 year Maternal Child Health nursing within in the past 5 years. Satisfactory work references.




Medium to heavy work. Constant handling of under 10 pounds to occasional handling of 70 pounds. Frequent driving required. Standing, walking, and handling are preformed on a constant basis. Sitting, carrying, bending, lifting/lowering, pushing/pulling with whole body and with arms are performed on a frequent basis. Squatting/kneeling, twisting, and reaching are performed on an occasional basis. Adequate vision and hearing to carry out assigned duties.
