The Patient Flow Coordinator in an integral member of the Hematology /Medical Oncology clinical program. The Patient Flow Coordinator will be responsible for facilitating administrative and clinical activities to successfully admit chemotherapy patients. The primary roles of this provider are interfacing with point of care staff, partnering with Bed Management and Y-Access, communicating admission plan detail to patient/family, physicians, APPs and others involved in the patient's care. This individual will serve as the primary admission contact for patients and providers.
Bachelor's Degree in Nursing required; Master's Degree in health related field preferred.
A minimum of three (3) years experience in adult inpatient services or ambulatory oncology service. Clinical expertise related to patient population.
Connecticut RN license
Demonstrated ability to communicate effectively both verbally and in writing, with all members of the interdisciplinary health care team to enhance collaboration and problem solving. Ability to incorporate into practice advanced clinical and theoretical concepts with patients, families, nursing staff, and other disciplines.
Provides daily management of all admissions to Smilow, coordinates the chemotherapy schedule and ensures authorization, monitors patients off tower and facilitates transfer to Smilow when appropriate, runs daily bed huddle and works directly with all outpatient areas, Y access, ED and the ECC and SREC. works with MD's to provide acceptance of patients to the correct location.
